Lakeshore is a not-for-profit organization that has provided important civil legal services for 51 years. Presenter is unable to offer individual legal advice to those in attendance.”

The FEBRUARY Meeting for the Healthy Partners C of C will be held on Thursday February 1st from 8:30 to 10 AM at the Madonna School of Nursing DiPonio Bldg. (Room 118), located on the NORTHEAST CORNER at the intersection of Levan and Schoolcraft (36600) in Livonia.

 

Wayne State University: Interprofessional Team Home Visit Program Fostering a Collaborative Approach to Patient Care

 

This program introduces Wayne State University’s medical, nursing, occupational therapy pharmacy, physical therapy, physician assistant and social work students to team care for older adults. Students assess an older adult’s health and social needs.

 

During this 60 minute home visit, students ask questions about daily activities, nutrition, medications, family health, and/or social supports. If an older adult has a third year pharmacy student as a member of the team, s/he will also receive a follow-up visit during which a medication calendar and medication recommendations will be discussed. This second visit takes about 30 to 60 minutes.

 

Over 450 older adults have been recruited for this program to help us teach our students. On average, 250 of them are assessed in their homes annually. The older adults appreciate the time they get to teach students, not only about their own aging but what it means to be a health care practitioner who is caring and willing to do home visits for those who may be homebound.

 

If you are interested in becoming a teacher of health care students or need more information about the program, please plan to attend this 30 minute presentation.
